Informed Consent
By using this app you agree in conjunction with your usage of the Driverly app, which we understand your employer has asked various employees to volunteer to use, as part of a Trial (here forth referred to as ‘the Trial’) between Ashwoods Lightfoot Limited (”Lightfoot”) and Driverly Company Limited (“Driverly”).
General information about the proposed Trial:

The purpose of the Trial is to provide an opportunity for Lightfoot to assess the quality of Driverly app and its telemetry data.

The Trial will require you to download the Driverly App and provide the necessary in- app consents needed for the app to function. Once that is done, the app will begin to collect telemetry data related to car journeys you subsequently undertake for the duration of the Trial. This information is then analysed by Driverly and used to assign your driving patterns a score based on your driving behaviour.

The Telemetry data we will collect comprises of your driving data including where, when and how you drive.

Other information collected during the Trial will include your email address.

This means that both Driverly and Lightfoot will be able to associate you’re the
telemetry data and the inferences made from it directly with you personally.

The duration of the Trial will be 6 weeks; at the end of and during the Trial, Driverly will share the
data we have collected and the inferences we have made with Lightfoot.

At the end of the Trial, you are free to delete the app from your mobile device. If you do
not do so, any information collected by the app after the Trial has elapsed will NOT be
shared with Lightfoot.

Participation in the Trial must be on a voluntary basis; if you feel compelled or forced to
participate in the Trial, then your Consent is not valid under General Data Protection
Regulation (GDPR) and you should not complete this form.

For the purposes of the Trial, Driverly and Lightfoot will operate as independent
Controllers of the data we capture and share.

Our Lawful Basis for collecting the data during the Trial will be Consent. Under GDPR,
this consent should be freely given, informed, unambiguous, specific and affirmative.

You are free to withdraw your consent at any point; you can do so by emailing us
(using the email address that you used to sign-up for the Trial) at hello@driverly.ai and clearly stating your intention to withdraw your Consent. Please note that simply deleting the app from your mobile device will not be sufficient - as data we have already collected through it will still be shared with Lightfoot unless you specify otherwise.

Once the Trial has finished, Driverly will delete all information collected during Trial from our systems In accordance with Driverly’s data retention policy.

Informed consent for the Trial

1. Taking part in the Trial.
I have read and understood the Trial information above, or it has been read to me. I have been able to ask questions about the Trial and my questions have been answered to my satisfaction.
I consent voluntarily to be a participant in this Trial and understand that I can withdraw from the Trial at any time, without having to give a reason.
I understand that taking part in the Trial involves downloading the Driverly app from the app store, giving the appropriate consent for the app to access my phone’s settings including but not limited to location. The app will ask for the required permissions at the first start.
I understand that by taking part in the Trial, there is the risk that Driverly may record and share details of accidents that may occur whilst I am driving and potentially, data that may incriminate me, if I am responsible for the accident.
2. Use of the information in the Trial.
I understand that information I that will be collected through the Trial will be used to assess the quality of Driverly app by Lightfoot.
I understand that personal information collected about me that can identify me, such as my name or where I live, will not be shared beyond the Trial team.
Your Rights to your data
Under the Data Protection Laws in the UK, you have a number of rights, including:
Right of access: You can ask us what information we hold about you and request copies of it.
• Right to rectification: You can ask us to rectify or update any of your information that you think is inaccurate.

Right to be forgotten: You can ask us to erase your personal information.

Restriction of processing: You can ask us to restrict the processing of your personal
information.

Right to object to processing: You can object to the processing of your personal
data.

We will not charge any fee for you to exercise your rights and will respond to any
requests under your Rights within one month of receipt.
